These forms are designed to help us work together to achieve your specific goals as a team.  Simply download them and fill them out and either email them directly to me or bring them with you to our first meeting. Many of these forms are fillable forms.

Physical Activity Readiness Questionaire

This questionnaire will tell you whether it is necessary for you to seek further advice from your doctor OR a qualified exercise professional before becoming more physically active.

Confidentiality Agreement Form

This form acknowledges your information will be used for fitness evaluation purposes and for the design, implementation, progression, and maintenance of an individualized fitness program only and not shared with anyone outside of our agreement

Medical History Questionaire

This form will help identify any physical or health limitations that could alter your training program.

Informed Consent Form

Exercise carries some risk to the musculoskeletal system (sprains, strains) and the cardiorespiratory system. and certify that I know of no medical problem (except those noted) that would increase my risk of illness and injury as a result of participation in a regular exercise program.

Comprehensive Client Information Sheet

You will be asked to provide some relevant personal information. The answers to these questions are essential in order to allow us to design an optimized individual fitness program for you.

Exercise History Questionaire

This form will help identify your current level of activity and will be used to structure a personalized training program specific to your experience, level of training and goals.

Health History Questionaire

This form will help identify your particular health, work, social and pleasure history and is helpful in structuring your particular training program

Medical Release

This medical release must be signed by a medical professional in the event you have any physical or health challenges for which beginning a training program may be contraindicated.
